The January crash of a military transport plane that killed 20 Polish Air Force officers, including some of its top brass, was caused by a series of procedural errors at all levels, a special commission announced this week, but pointed to the pilot's decision to bank the machine sharply on approaching the airfield at Mirosławiec as the direct cause of the accident.
